Although technically, the doctor fills out the form, not Homer, I think. Incidentally, other illegal activities were selling alcohol before 2 PM and rebroadcasting MLB with implied oral consent, not express written consent.

I can't recall if it was Homer or the doctor who actually filled out the form, but he definitely responded with that explanation. I was thinking about mentioning the rebroadcasting, or even the bullfighting to round out the animal-themed illegal activities, but tried to stick with potential crimes that occurred on Mr. Burn's yacht itself.
