I got a lcd with no dead pixels on the first try but many people I have spoke to have had bad luck. One friend is on his 5th lcd from circuit city and they all have had 1 dead pixel.
My understanding a dead pixel is due to a bad transistor and nothing can be done but some people claim that sometimes you can massage a dead pixel back to life. I would not count on that method working though.
