I don't think that they'd expect you to have your specialization decided already, so if you're not sure about that just don't write about it.
Of all fields you could go into, why audiology? How does the field appeal to you? Do you have an idea of what careers in the field are like and why would you enjoy them? What exposure, if any, do you have in the field? Do you know anyone, and if so, how has their experience motivated you?
I imagine answering these questions would lead you towards a nice one-page letter.
