One time at Fuji Do in Harrisburg (highly reccomended. go and see Twan if you can.)
I got ama ibi, and there it comes with the deep-fried heads of the shrimp. I didn't eat them an the waitress came and said, "You didn't eat that? it's the best part." I thought it was a game--get the round-eye to eat the garnish, but she was serious. I ate them. Tasty. The second(long) antennae are a bit awkward, but it's neat.
