"Not too bad.. listen, I honestly (make sure you use that word) don't remember your name. I feel really bad, and I'd like to make it up to you. How about I take you out to dinner?"

or substitute with something else. but just do it straightforward- you're sink or swim on this, so just go for truth. if you lie and she buys it, how perceptive/bright a girl is she, anyway?
