i think he imagined the car accident... he's not a particularly happy person, atleast in the beginning of the movie, and this probably demonstrates it.
the piano... I don't want to get all symbolic here, because I'm usually wrong. but it seems to bring some peace to his life... he seems to go to the piano when things get hectic, atleast that's the way i remember it. this probably represents something missing from his life, and I think that the girl he finds is very similar to the piano in terms of their impact on his life.
