I am usually burning a full cd, but the fastest I burn at normally is 48x. I did burn one (the first one, that came with the drive) at 52, but havent seen nor therefore bought any more 52x media. in any case, the speed doesnt seem to affect the noise. Its loud no matter what.
as for case noise, believe me, Ive heard some loud fans, but then, I'm someone who sleeps with a fan on, coz I like the way white noise drowns out most other sound. So I guess its just a matter of perception.
