I've had it since birth all the way until I was about 17. My couple of trips to the doctor involved them forcefully pulling it back with no anesthetic. Let me just say that I've become very tolerant of all kinds of pain since those visits.
The maintenance since then simply required me to pull it back everyday to keep skin stretched. I didn't because I grew up not doing it. It wasn't a habit. I broke it by getting a girlfriend when I was 17 and frantically trying to get it in "working order" so that she wouldn't think it was weird. So to answer the second question first, I have no idea if it has an effect on sex since I've "cured" it before I started having it at that age.
To answer the first, while it is possible to have an erection with the skin around the base, it will hurt. At least it did for me. If it's not stretched enough, I don't want to say for sure, but there
might actually be some tearing. I'd wait until you can freely move it up and down while flaccid without any hangups. I had the same problem with the skin not going back, but it corrected itself once it's become a regular routine for me. Just stick to pulling it back daily and you'll be set.
