Pretty much the same whenever I've done this. I always maintain eye contact no matter what position. If the girl closes her eyes/goes limp/taps my arm/flails or makes any sort of erratic motions I stop. Always err on the side of caution, especially early on. The choker can obviously get a thrill from the power/control involved, but every focus has to be on the chokee to prevent injury/death.
Something I would never mess with early in a relationship, and never mixed with intoxicants.
NEVER use props. The potential for injury is just so much higher. Props, like any simple machine, can amplify the power of the user. You may be able to cause some real damage. I can imagine it would difficult to go into work the next day and explain that nifty garrotte bruise.
