This doesn't seem to be so much a dentist-specific question as a general small business question and potentially a specific legal question.
The real question for me is why there is this credit. I suspect that it is there on purpose to cover postprocedure care/checkups. Now if you don't want that care or can't afford to have the credit sitting out there, you can ask for it back. They may or may not give it back to you. If they don't, I don't see how you have any choice but to litigate in small claims court.
I think that you need to call their billing department and find out why this credit is there and if it is refundable. If it is not, you may have free dental care for a while.
