Typically in Florida restaurants I've found that they soak the onions in a little seasoning and some balsamic vinegar with a teeny bit of olive oil and garlic. Then they sprinkle the top of the tomato with a little bit of salt, then the onions (drained a bit) topped with the bleu cheese or gorgonzola cheese. I've also had it flash broiled so the cheese was slightly melty and a tiny bit brown. Yum!
