I've never had a problem.
You don't have to piss sitting down. You just have to be more vigilant about dripping.
As for infection, with proper care, there is very little chance of infection. I did get a slight infection but some polysporin cleared it up in a day or two.
As for the thread catching... never had a thread yet on the PA though I did have them catch on an ampallang and that fucking hurt.
