It happens when the station or cable company you are watching does not properly synch it's local feed with the national feed. Local stations and cable companies are given blocs of ad time to sell for local/regional ads that they are able to run over the ads supplied by the national feed. That's why you will see a few (but not too many) ads for local businesses during events like the Super Bowl. These businesses didn't pay $1.5 million like Pepsi and the others. They paid whatever the local rates are.
Anyway, if the feeds are not synched properly you see the 1 - 2 seconds of the national ad before it swithches over to the local.
