One thought that came to my mind is that often women may get perineal tears in childbirth. The recommendation that I've heard is that they refrain from sex for about a month afterwards for things to heal. Have you tried Lanolin. It is great for healing. I used it on my nipples while I was nursing because it's not bad for you to injest small amounts. My nipples were constantly being irritated and the cracks in the reopened but the Lanolin kept the skin soft helped protect the cracks while they healed from the inside out.
Having said all that I strongly suggest as other's said here that you should go to another Dr. If it's too late to file a malpractice suit you could at least write a letter of complaint to the clinic that your previous Dr. works for. They will probably supervise her work a little closer because they'd rather avoid future malpractice suits I'm sure. It would be doing SOMETHING but not causing a long ordeal.
Hope it gets better soon.
