-Try different resolutions before launching the game. I found the game was less than graceful dealing with resolutions it didn't like at game launch.
-Quit your video card's taskbar resolution tool. It may be auto-switching you into an unusable resolution/refresh combo. (As a troubleshooting step. Once you find the problem you won't have to jump through hoops.)
