i had a cousin who didn't realize she was pregnant, despite having 5 children previously. noone had a clue until she died and the autopsy revealed she was about 7 months along. she wasn't in denial and trying to convince herself she wasn't pregnant, she simply didn't have any symptoms like she did with her previous children.
i've known several women who had no idea until they started to show. one woman actually thought she had cancer (though in her defense, both her and her husband had been "fixed" about 10 years prior).
and some women don't show much, if at all. one girl i work with kept telling everyone she was pregnant--but she didn't show at all. and when i say she didn't show, i mean she did not show--everyone thought she was making it up. she came to the hospital to give birth wearing her size 6 jeans.
