You said that the condom came off prior to ejaculation, and that makes you safer than most. But maleficent is correct, certain antibiotics have been proven to interfere with the hormones which prevent ovulation (which is what the pill does). Your girlfriend's doctor probably told her that her birth control may be rendered ineffective by the antibiotics when they were prescribed.
Whether or not there would be a risk of pregnancy would depend entirely on what part of the ovulation cycle your girlfriend is in, but as was said, emergency contraception is the smartest idea at this point. No need to tempt those odds.
