Get the Microsoft InteliiMouse Optical.
It's got 5 buttons, a scroll wheel (a must) and it just fits any size hand (left or right) like a glove. Plus you can usually get them pretty cheap.
The best feature (IMO) is the back and forward buttons, perfect for web browsers.
