I have to agree with Bobaphat and go against the tide here. If you have a reasonably good relationship with her I would advise you to tell her tactfully. Let her know her underwear is visible at certain times and that you wouldn't want her to become embarrassed in front of other employees. She will appreciate you looking out for her interests and if it was intentional she will have the opportunity to let you know. I have a good working relationship with women in my office and the few times I have told them about their garment malfunctions they are usually appreciative.
