As diergray said, there was one failure. But most of the time, these guys don't really complete everything they've set out to do; they just reach a point that the producers accept as complete on the major projects; sometimes that means that a device that they put in the wall (the swing-out disco ball comes to mind) is installed and working, but they overlook the little detail that that the hole they tore in the wall still needs to be sheetrocked and plastered.
I 'm pretty sure that the production company has a clean-up crew of construction workers standing by who come in and finish the details (and also to finish the job if the team of the week screws up). Then the decorators move in.
