Still even if the pot wasn't fresh more than likely it was kept on a warmer to keep the contents hot. I'd think most people would know better than to put a cup of something that hot in their lap because something like this could happen.
Most people I know buy coffee because it's hot and they blow on the contents to cool it off before they drink it.

The coffee wasn't "fresh". You don't drink next to boiling water, do you? 180-190 degrees is still too hot to drink.

McDonalds also said during discovery that, based on a consultants
advice, it held its coffee at between 180 and 190 degrees fahrenheit to
maintain optimum taste. He admitted that he had not evaluated the
safety ramifications at this temperature. Other establishments sell
coffee at substantially lower temperatures, and coffee served at home is
generally 135 to 140 degrees.

They keep the coffee that hot, on purpose.
Something like what could happen? Usually coffee isn't hot enough to give you third degree burns in two to seven seconds. If the coffee would have been just a few degrees cooler she might not have gotten those burns, not to taht degree anyway.
Any suggestion where she should have put her cup, in a car, when trying to open it to add milk and sugar?
