don't have a bad accident to share but.....
my girlfriend and I (this was 3 1/2 yrs ago) we doin it doggy style. We like to cjange up a bit in positions so I was leaning down and she was almost on top of me backwards. I was done soon after that and right after I pulled out.....
she farted
it was a loud one too, I didn't laugh at the time and all she said was "woops." I felt too good too laugh but when I think back about it, I usually fall off my chair
