The majority of my injuries have been from elbows to my head while changing positions. Painful, but nothing horrible.
Though, one time I was with a girl who was very enthusiastically riding on top of me. Her knee ended up slipping off the side of the bed (mini sized college bed) and she slid off, taking me with her. The change in location hurt our pride more than anything, especially when the crash woke up her 3 room mates. oops.
Unfortunately, it didn't turn into an exciting Penthouse letter event.
