Not to TJ or anything, but . . .
Buried alive seems to be a recurring choice.
I read once that, upon examination of many old caskets in a European graveyard (they had to be relocated) that over 1/4 of them had scratchmarks <i>on the inside</i> of the lid - a sure sign that the person buried had recovered consciousness and tried unsuccesfully to escape. I guess in those days they were more likely to bury you if you had your eyes closed too long. In fact, some caskets had strings attached to bells on the surface . . . just in case.
Anyway - if you believe in reincarnation, this would explain a lot of claustrophobia cases, eh?
