Personally, I'd prefer it if the prices for the food already included enough for the servers to receive an actual salary, and tips would be just if people felt like it. I mean, I don't tip the people at the movie theatre or the clothing store.
But, that's not the way the USA operates, so I default to 20% unless something has gone horribly wrong, and the fault lies with the server.

Groups of six or more are generally charged gratuity in the US. There are many reasons behind this but the fact is that servers make their money on tips and if they aren't getting them they will move on to to a restaurant where they will. Can't say if it is fair or not, but its usualy printed on the menu, right next to the no splitting bills that the rube's like to ignore.
