From what I have heard it happens prior to any sperm being released as a way to sort of prep the urethra and make sure the environment is suitable to your swimmers. So even if you don’t notice it is still there right before you come, but in an extended session it is just keeping the urethra ready the whole time so you tend to notice it more.
