The fact that it hurts now 6 months later and is very sensitive to pressure and heat makes it likly that the infection has gone into the pulp (nerve space). You say that it was a very deep & old cavity to begin with so this makes it even more likely. An X-Ray and a couple small tests and your dentist will be able to tell you if the pulp is infected. If it is, you'll need a root canal. If it's not a front tooth, then you'll also need what's called a "post & core" along with a crown. See my post from 6/16 for general prices.
