Read this: The FBI Files: Vaginismus | Scarleteen. Vaginismus is a sort of sketchy thing to point to as a cause for this, because it's easy to turn "psychogenic" into "you're making it up". It's a real condition, it's just that it's of mental origins.
BTW, that site, Scarleteen, is a very good resource for real-world sex ed stuff that they never taught you in high school health class.

I would guess it was this as well.
If she fears sex (even unconsciously), the muscles down there won't relax.
There may be some other reasons, but the above should cover a large percentage of these cases.
