That one was easy - I dont see how it can go wrong.
With Jack, I figured it was because I sided with Miranda - I didnt have enough paragon/renegade stats to open the reconciliation option with Jack.
With Zaeed, I chose to save the people instead of chasing his former partner.
With Thane, I somehow lost the trail of his son's mark. I actually couldn't see a way to follow, so i figured it was part of the story.
