Over mine and my sibling's childhood years we had some creative ways to pull our teeth.
It usually involved string and stationary objects, or my father and his greasy pliers. (mmm, tasty)

One time, I think I was in 3rd grade, a bunch of us were playing touch football after school while waiting for our rides.
A friend went for a long one (long for a kid) and collided with a metal pole supporting a covered sidewalk. He chipped his tooth on the pole and went and had it glued back on.
You could tell ever after where it broke off.
My last tooth to come out was a wisdom tooth, about a year and a half ago.
A couple of pieces started breaking off so I went and had it x-rayed.
