The active ingredients are Niacin (increases blood flow to the clitoris) and Propylene Glycol (warming sensation).
Propylene Glycol is unfortunately in some women an irritant, and can lead to a burning rather than a warming sensation.
All in all, this stuff works for around 3/4 of women tested.
Bearing in mind that the way to get it going is to massage it into the clitoris, over half of the effect could easily be attributed to the placebo affect and to the masturbation.
The actual cost of the ingredients in this product is very very low - no more than 10c for a 50ml bottle would be my guess, so the price tag they put on them is massive.
It's a good profit line to the retailer, and to the maker, so you can see why they promote it so hard.
