For the jerking:
Replace fuel filter, vacuum leak (use a can of carb cleaner and spray around the hoses, if engine speed picks up you have leak.) Replace spark plug wires, distributor cap and the rotor.
For the transmission issue, try changing that filter. It's a little bit of work, but is always forgotten.
