In my experience, people who have gotten their tits done like them handled like they did before they got the surgery, with some possible loss of sensitivity on the nipples.
As far as paying attention to them or not... they paid thousands of dollars to modify their body in a way they thought would make them more attractive. Of course they want you to notice! They didn't do it just for themselves.
