#1 is people who don't get how traffic circles work, and stop before they enter. I actually had someone who was already inside the circle stop to let me enter in front of them... thankfully there was nobody behind them.
A close second happens at least 5 times every time I drive through Quebec: I'll be driving along at a good speed in the left lane, and the very slow driver (relatively speaking) in the right lane waits until I'm 5 seconds from passing them to pull in front of me and sluggishly pass another car on the right.
