Make a list of things that she likes. Her favorite things (movies, skydiving, ballet, rodeo, dog show, restaurant, beach, you know the answers better than I do). Figure out which one(s) you can pull off. Go from there.
I agree with Albania about making a day about it. Whatever you decide to do, a momento or two would be nice to remember the special day you planned.
Photos, postcards (if from the beach or other outing). Let the restaurant know ahead of time you would like to keep/purchase the wine glasses. If that isn't possible, bring your own. Make them pretty with ribbons on the stems. On your outing, buy a bottle of champagne, wine, sparkling juice, or whatever you two drink...to be saved for your next anniversary along with the glasses you just used. Above all, make it personal.
