I think that's really the only thing people are saying here. I don't think anyone is suggesting that everyone MUST go to a hospital and be plugged into a thousand machines and put on all manner of drugs. All we're saying is that it's a much, MUCH safer idea for mom AND baby to have someone with formal training there in case things go wrong.
As for episiotomy, the choice to get one is yours, not your doctor's- so "I don't want birthing with a doctor because I don't want an episiotomy" makes no sense. You choose the limits of your medical care as long as you're conscious and sound of mind to do so.
You do not have to consent to any manner of treatment that you don't want, up to and including interventions that would prevent your death.
