I doubt they're gone for good. Despite this episode's emphasis on change, House the series generally has very little change. Vogler, Tritter, House getting shot and getting his leg working again all resulted in the status quo. Aside from some subtle personality changes like Cameron being less sweet and naive and Chase actually contributing something, House and co is still the same. Plus Omar Epps said in an interview that he would be back next season.
That said, I really liked this episode. It showed House being a little more human, or at least trying to when he asked Foreman to stay and smoking the cigar with the Cuban. It's also nice to see Cameron and Chase together since they're engaged in real life.
