Use the cover letter to explain what she's doing in her current position.
Use the resume to describe what her duties are. She's probably not a full time case worker, but if that is some of her duties, add it. A title is just a title, you can call yourself whatever you want it's all about how you market yourself.
She can either ask for a raise or she can't. She can ask for a title change or shecant. If she doesn't ask she won't get it. The company will either value her as an employee or they wont is what it comes down to. She won't get it unless she asks. Will they replace her? Depends on how difficult she would be to replace, but
If I remember correctly, Non Profits are exempt from a lot of the employment laws that cover other companies...
