A full body massage generally implies full back, calves, thighs (though not too high up), arms and neck and sometimes head. I have had a few that have done my stomach (never my chest) and one went a bit of the way down on my butt (the glutes) but it was nowhere near arousing. I have never had a male masseuse so cannot offer any insight there and so therefore have never once felt aroused while getting a massage.
As for being naked - they generally want you to remove your bra to make it easier for them but you can leave your underwear on if you're more comfortable with that or take it off, whichever you want. A professional masseuse will only do what YOU are comfortable with and nothing more!
