You just need one hand and a mouth. If traffic gets tight, you can leave the harmonica in your mouth and just breathe in and out.
We definitely keep folks around us laughing, that's for sure.
If she's in the car, I get her to play NAME THAT TUNE on the SIRIUS radio. She can't look at the display, and she has to name the song and artist as I move upward one station at a time. When she's bored with that, she'll start saying "Are we there yet?" repeatedly and telling me about all the things that ALMOST just hit us.
