Sony Ericsson Z520A. It's a great phone with excellent reception and battery life, plus camera and bluetooth. Built in media player doesn't hurt - though it's not a true mp3 player, like some others.
Motorola v557 - camera, bluetooth, and a Motorola... or...
If you want to save money, a v551. Exactly the same phone with slightly older software (hence a dropped call here or there, though not many), and no "live ticker." So... who cares? These can be had relatively cheap these days off of ebay, even new.

Edit: Forgot to mention: there aren't a lot of inexpensive Nokias that have the features you're looking for. Generally the higher end ones (read: >$400) have bluetooth, but they are also PDAs or have integrated keyboards, which is what makes them more expensive. The lower end ones don't usually.
