My wife had hers done and i think all of your concerns can be covered by good preparation.
A: Don't go nuts... If you have an A cup do not shoot for double D's. If you are small and plan on going large, plan on more than one surgery. Make it a two step process and give your body time to adjust and accomodate.
B: Under the muscle not over. This keeps your own breasts on top of the implants helping to cusion and keep a natural feel.
C: Quality over cost. Much like auto repair, the money you save now aint worth the money it will cost you later. Horrible boob jobs can haunt you forever and may never heal right.
D: Bra's. Wear them all the time, well except on special occasions. Sports bras at night and non-wired in the day time. Bras will keep the implants on your chest and not on your stomach. The more you wear a bra the longer they will stand at attention.
While these tips will not keep everything from going wrong, if you get a quality surgeon, they will lead to many years or trouble free titties.
for the final question, FEEL. I like the feel. In fact i would go as far to say i like the way they feel more now than before the surgery. I have felt some that were not taken care of and they were like rocks.
