I'm just skeptical because I've seen lots of videos made "with the graphics engine" that looked nothing like the game when it actually came out.

If this was any other thread I would be agreeing with you, but this is MGS4. Hideo Kojima has constructed all four game trailers for all of his recent MGS games using the in-game engine. MGS1's trailer was comprised of actual game cinemas. MGS2's was made with in-game cinemas as well, with a little bit of Kojima humor thrown in. MGS3 had a mixture of cinemas made for the trailer and actual game movies. MGS4 looks like it will either be part of the intro of the game, or just a teaser made from the game engine. Kojima has never released a trailer that wasn't the pure gameplay engine, and I don't see why he has reason to now, since he thrives on making a big deal at gameshows (people were talking about the MGS1 and MGS2 trailers years after their release).
If this was run on a sped up dev kit, it'd be all around the Internet now like the Killzone movie.
