Is the file on your desktop?
I regularly see that problem with stuff like Norton AV updates where people have saved them to desktop and then run them.
If you have stuff saved there, windows likes to 'read' them when you refresh to desktop when minimizing all open windows. I had to kill the Norton AV service in order to delete it.
