Wow, what an interesting thread! I've been thinking of getting my sixth tat and just read through everyone else's input.
Short and sweet, my first is a custom tribal around my ankle, several years ago; my artist told me that's one of the most painful places but while he was doing his work I was looking through a magazine and already planning my 2nd.
Felt most like hot cat scratch at times where skin is thinnest over bone, mostly during outline phase.
Have one on each shoulder blade, only felt the scratch a couple of places on each one, just felt gentle pressure the rest of the time.
Recommendations: In addition to earlier comments about picking something you can live with and reputation/cleanliness of facilities, be sure to take good care of your work afterwards. Also, and I know I may catch some flack for this one, if you plan on ever working in a corporate/business environment, be sure your tat is someplace where you can hide it while at work. Unfortunately, regardless of a spotless reputation and business practices beyond reproach, there are still lots o'folks out there who will judge people negatively because of a tat or piercing.
A note about piercing . . . when I got my belly ring, after five decent sized tats, i was so nervous i nearly passed out! Have no idea why, since tat is so much more of a permanent commitment (and a much longer process than a piercing)!
I may post pics of my work in the place amonkie mentioned :-)
