First off... some guys wouldn't know an orgasm if they tripped over it... so faking it is easy.
As for men who are actually attuned enough to know if a woman has orgasmed or not... they are probably skilled enough that faking isn't even an issue.
Personally I would hope that no woman would be so disrespectful as to fake it for me... I'd rather they said, "Don't worry, I don't think I'm going to get there this time... give your jaw a rest," or "Fuck your jaw aching... get back to work!"
