Soviet Russia had nukes for years...thus the cold war

as far as what would happen, it depends on the proximity to the blast. The area close to the city (those directly affected by the blast) would probably panic. Other large cities might panic as well, and cities/towns close to military targets would be afraid as well. Areas further away (small farm towns, places in the mountains, etc.) would be less affected. Some of the hard-to-reach places might not be affected at all.

This scenario draws on the experience of 9-11.
