Countless hair in food stories here. The worst are (suspected) pubes in food. I just think 'wth was going on back there'.
Otherwise, not any I can recall. Ordered a medium steak once and was given an almost blue-rare. That was gross at the time (nowadays I'd just eat it, mmmmmm, steak).
