It actually doesn't hurt at all. You can't even feel the needle go thru. The only uncomfortable part is the pinch from the clamp and a quick burn for about 5 seconds when the jewelry is put in from the alcohol cleaning solution on it.
From the time its in, I haven't had any pain at all, except once when I was stupid. I made the mistake of waking up and just throwing on a swimsuit to wander around the house in. The netting inside happened to be the perfect size for snagging the end of a barbell! Youch!
It's actually really easy to keep clean, and really causes no pain, and the benfits are definetely worth it. I'm just debating between the ladder, and the stairstep at the moment.
