I've tried using the Aneros a few ways while having sex with my wife and found that the best way is with me on my back and her on top... The Aneros itself isn't all that big and doesn't get in the way at all in this position.
The beauty of the Aneros isn't in the size of the thing, rather it is in its ability to rest exactly on the prostate... Don't worry about the curly bits... one part rests against the perenium (more pressure on the prostate) and the other sits between your cheeks...
In my experience, having that pressure on my prostate while I orgasm creates a very intense orgasm... A lot stronger than just a regular orgasm.
